Anemia after bariatric surgery: Can we predict it before and early after surgery?

Learning Objectives:

  • Recognize anemia as a common long-term complication following metabolic and bariatric surgery (MBS), even in the presence of relatively stable hemoglobin levels early after surgery.
  • Identify key preoperative predictors of long-term anemia, including advanced age, elevated fasting blood sugar, and higher triglyceride levels.
  • Understand the role of early postoperative weight loss—particularly at 12 months—as a predictor of future anemia risk, with greater total and excess weight loss associated with increased likelihood of deficiency.
  • Interpret receiver operating characteristic (ROC) metrics, including AUC, sensitivity, specificity, and clinically relevant cut-offs, to stratify patients at risk.
  • Apply a risk-based approach to postoperative monitoring, enabling earlier identification of patients at higher risk for anemia.
  • Integrate findings into clinical practice by implementing targeted nutritional surveillance and individualized supplementation strategies to mitigate long-term hematologic complications after MBS.

Dr. Erfan Sheikhbahaei is an internationally trained physician and surgical researcher with a strong focus on surgery and clinical outcomes research. He graduated summa cum laude from Isfahan University of Medical Sciences, Iran and has authored over 60 peer-reviewed publications across multiple surgical disciplines.

He is currently a Research Assistant at University Health Network (Toronto General Hospital), where he contributes to advanced clinical data analysis and outcomes-based research. His work in bariatric surgery includes studies on postoperative complications, nutritional deficiencies, and national trends in obesity surgery, with a focus on improving patient outcomes and surgical quality.

Dr. Sheikhbahaei has presented his research at international meetings, including International Federation for Surgery of Obesity (IFSO) conference. His academic interests include metabolic surgery, surgical innovation, and the multidisciplinary integrations into clinical research and healthcare systems.
